0008212: theme header references missing images

Description

After a fresh install of phplist, the theme header for the "subscribe" frontend page has several broken images at the top/left and top/center of the header.

** The broken images icons are only visible in Internet Explorer, not Firefox. In Firefox the images are simply missing.

The source for the header is taken from:

/lists/admin/defaultconfig.inc

The images in question are:

top01.png
top02.png
top03a.png
top03b.png

Those images have not been included with phplist for a long time.

Editing that file after installation makes no difference to the subscription page, so it must be using a copy in the database or somewhere else.

Additional subscription pages created via the backend tool also exhibit the same problem.
